A century ago, the discovery of mutants came about in a terrorist known as Smog who had the power to produce smoke that caused Tuberculosis. Now, over a century later, those with superhuman abilities are known as Mutants. Mutants make up less than 1% of the world's population but are often the talk of people who either feel mutants are monsters or weapons that haven't been used yet. The US Government have an uneasy relationship with mutants, but a certain mutant a -- Adelaide McDowell, known as Phoenix who is known to be rich and powerful decided to fate against the hatred and create a school for Mutant youth to train them in their powers and show the world that mutants aren't a danger.

La bataille contre Xehanort appartient désormais au passé. Grâce à l'union des sept Gardiens de la Lumière, les ténèbres ont été repoussées et le Royaume des Cœurs a retrouvé son équilibre. Pour la première fois depuis longtemps, les mondes respirent en paix.
Contrairement au destin qui semblait l'attendre, Sora ne disparaît pas après avoir sauvé Kairi. Son retour défie les lois qui régissent les mondes, mais personne ne peut expliquer pourquoi. Même Yen Sid, Merlin et les trois Bonnes Fées peinent à comprendre ce miracle. Sora est vivant... mais quelque chose a changé.
Peu à peu, des phénomènes étranges apparaissent. Des corridors obscurs s'ouvrent sans raison, des fragments de mondes oubliés ressurgissent, et certains habitants affirment apercevoir des lieux qui n'existent plus. Les frontières entre les mondes deviennent instables, comme si une force invisible réécrivait lentement la réalité.
Les Sans-cœur et les Simili réapparaissent en nombre, mais ils ne sont plus les seuls à menacer l'équilibre. De nouvelles créatures, nées des émotions oubliées, des souvenirs effacés et des possibilités qui n'auraient jamais dû exister, émergent dans les interstices des mondes. Elles semblent guidées par une volonté inconnue.
Afin de protéger les mondes, Yen Sid convoque une nouvelle génération de Porteurs de la Keyblade. Certains sont des héros déjà connus, d'autres n'ont jamais manié une Keyblade auparavant. Tous devront voyager entre les univers afin d'enquêter sur cette mystérieuse perturbation.
Car une vérité inquiétante commence à se dessiner...
Le retour de Sora n'était peut-être pas un miracle.
Il pourrait bien avoir ouvert une porte que personne n'aurait jamais dû franchir.
Entre Lumière et Ténèbres, une troisième voie semble désormais émerger, prête à bouleverser les fondements mêmes de l'existence.
